Quilt Studio Intake Form

This is my personal intake form and it works well for my studio.  Feel free to use it if it works for you or just take some ideas and add your own personal needs to it.

After printing cut the page in half and you will have two complete forms ready for use!  

The top half is filled out upon intake and the bottom half is for use in your studio to measure and track how many bobbins you use etc... 

I included a space for 'extra charges' use this for your design fee, loading fee, or if you had to fix seams etc....
